About this service

What does Post-hospital psychiatric follow-up include?

An outpatient follow-up reviews discharge recommendations, medication changes and your current needs after acute care. Bring discharge paperwork and an accurate medication list so Younetic can assess whether the practice can provide the level of follow-up you need.

This pathway may support adults transitioning from a psychiatric hospital, emergency department or other acute setting when routine outpatient care is appropriate.

Younetic provides adult psychiatric care in person in Niceville, Florida, and by secure telehealth for eligible adults physically located in Florida, Oregon, Washington, Colorado or Arizona. Availability and the appropriate care format depend on clinical needs, location, prescribing rules and insurance requirements.

Your care may include

  • Discharge-plan and medication reconciliation
  • Review of current symptoms, functioning and safety
  • Clarification of outpatient follow-up needs
  • Coordination guidance for appropriate ongoing care

The exact plan is based on an individualized assessment. Not every element is necessary for every patient, and additional information may be requested when it improves diagnostic or treatment safety.

What to expect

A thoughtful, individualized process.

Bring discharge paperwork and current medications. Younetic reviews what changed during acute care, identifies immediate outpatient priorities and determines whether the practice can safely provide the needed level of follow-up.

Younetic explains recommendations, answers questions and identifies appropriate next steps. Additional records, testing, medical evaluation or coordination with another professional may be requested before a diagnosis or prescribing decision is finalized.

Appointments consider symptoms alongside sleep, physical health, current and past medications, substance use, stressors, safety, daily functioning and the outcomes that matter to you. This wider view helps avoid treating one symptom in isolation.

Important: This service is not crisis care. Call 911 or 988, or go to the nearest emergency department, for immediate safety concerns.
Ongoing care

How are decisions made?

Care is collaborative. When medication is considered, the discussion includes potential benefits, common and serious risks, side effects, alternatives, interactions and monitoring needs. A recommendation is not a promise of a particular prescription or diagnosis.

Follow-up visits look at both symptom change and everyday functioning. The plan may continue, change or include coordination with primary care, psychotherapy, testing, specialty treatment or a higher level of care when those options better match the clinical need.

Prepare for your appointment

Have an accurate medication list, pharmacy information, relevant medical and psychiatric history, and prior records when available. Write down your main concerns and questions so the visit can focus on what is most important.
